Setting Up a Clinic? A Medical Equipment Checklist with Tuttnauer Autoclave Lessons

You're probably making equipment mistakes before you even start

Since 2019, I've been responsible for equipment purchasing at a four-provider primary care clinic. Eleven significant mistakes and roughly $18,000 of wasted budget later, I now maintain our team's buying checklist so nobody else has to repeat my education.

Step 1: Get the autoclave right the first time

In August 2020, I ordered a refurbished autoclave from an online marketplace to save roughly $2,000. The upside was $2,000 in savings. The risk was buying a used unit with an unknown maintenance history. I kept asking myself: is $2,000 worth potentially losing a week of sterilization capacity? It turned out the answer was no.

The unit failed a biological spore test within a month. The repair estimate was $1,400. The replacement cost was $2,400. Total loss: $3,800. Sterilization equipment is not where you cut corners.

Here's what to check before buying any Tuttnauer autoclave:

  • Chamber size vs. your largest tray. I bought a chamber that was two inches too shallow for our surgical trays. We had to buy new trays to fit — $620 wasted.
  • Cycle presets. Verify the unit has the cycles you actually use (unwrapped, wrapped, liquid, vacuum). Some models need programming modules for certain cycle types.

For what it's worth, we've had the best results with mid-range automated upper-arm units in the $100–250 range (based on online supplier price lists, 2025; verify current rates). The ultra-cheap ones are honestly not built for repeated clinical use.

Step 4: Mobility scooters — the purchase everyone forgets until someone needs it

We don't sell scooters, but we provide one for patients who need help getting from the waiting room to the exam room. In 2022 I bought one sight-unseen. It was, to be generous, too wide for our hallway.

Worse than expected.

Here's the checklist we now use:

  • Measure the narrowest doorway the scooter must pass through. Our unit needs 24 inches of clear width, fine for 36-inch doors, not fine for a standard bathroom door.
  • Check weight capacity. A heavy-duty scooter with 350–450 lb capacity covers most patient populations. Know your population before you buy.
  • Turning radius: this is the spec that gets overlooked. A 40-inch vs. 50-inch turning radius is a big deal in a crowded waiting room.
  • Battery range: for in-clinic use, range barely matters. For patients crossing a large campus, it's critical.

Honest limitation: if your facility is tight on space, a standard wheelchair may serve you better. Scooters are right for some patients, not for every situation.

Step 5: Digital radiography — what it is and what I wish I'd known

Digital radiography (DR) is an X-ray method that uses digital detectors instead of traditional film and screens. The image appears on a monitor in about ten seconds. No film processing. No darkroom. That part is amazing.

The part that wasn't amazing: I budgeted for the detector but not the software integration with our EHR/PACS system. That integration cost $4,200 and took five weeks of back-and-forth between two vendors who were happy to blame each other.

What I'd tell you before you buy:

  • Verify EHR/PACS compatibility in writing from both vendors before you sign anything. Verbal “should work” isn't a plan.
  • Budget for staff training. Even experienced X-ray techs need time to learn a new digital workflow.
  • Consider a portable DR detector. We bought a fixed detector in the table. I'm honestly second-guessing that choice when I see how often our mobile X-ray machine gets used.

On pricing: our detector plus software package ran $60,000–75,000, based on quotes from two vendors in 2023. Prices vary wildly by brand and configuration. Get at least three quotes. Honestly, I'm not sure why the quotes varied so much. My best guess is that software integration costs get buried in different line items depending on the vendor.

Step 6: Build a documentation system on day one

All these lessons pointed to the same conclusion: we needed a shared record system. In Q1 2024, I started a simple equipment tracking spreadsheet (now a small database) showing purchase dates, warranty expirations, PM schedules, and vendor contacts for every machine we own.

Why does this matter? Because one autoclave needed a firmware upgrade that should've been covered under warranty. We didn't have the proof-of-purchase paperwork because the person who bought it had left the clinic. Out-of-warranty service call: $300, entirely avoidable.

Step 7: Train staff on the actual unit you bought

Mistakes I still see people make

  • Buying equipment before checking electrical, plumbing, and physical space constraints — the $1,800 electrical upgrade and the too-wide scooter were both preventable with a measuring tape.
  • Letting one provider's personal preference override a written protocol — the wrist-cuff monitor incident, enough said.
  • Ordering equipment without a documented service plan. Even the most reliable autoclaves need preventive maintenance. Know who will do it and when.
  • Trusting a vendor's “there's nothing to it” without getting integration details in writing. That $4,200 EHR integration fee was the price of that lesson.

Here's the thing: no one avoids every equipment mistake. But a documented checklist and a culture of reviewing purchases before finalizing them prevents the catastrophic ones. It's the difference between an $18,000 education and a modest one. The mistakes I made in year one were expensive, but they're also why we have a functional buying system today.
